Do not include patient names, medical records, diagnoses, or other patient-identifying health information.

Does submitting the form confirm acceptance?
No. The form starts a conversation. Acceptance depends on the type, condition, expiration and eligibility of the supplies.
Can you arrange pickup?
The form asks about availability and transfer preferences. Coordination happens after review; no pickup date or schedule is guaranteed.
What if I’m unsure about a category?
Describe the supply in the form’s “Other” option or email clinicalbridge.atx@gmail.com. Please do not send patient information.
Can I give money instead?
Yes. Financial support is separate from supply donations and goes through our fiscal sponsor. Visit Support Us.
